How you will change lives
Case Management
Ensure consumer involvement in the Person Centered Planning Process (PCP).
Facilitate the implementation of Individual Plan of Service.
Perform comprehensive, ongoing psychosocial assessments.
Participate in legal proceedings including petitioning and testifying in clinical emergencies.
Meet with assigned persons-served face-to-face as deemed medically necessary.
Coordination of Care
Facilitate the coordination of consumer intakes, discharges, and transfers.
Communicate with all service providers in Treatment Plan.
Facilitate hospitalizations, attend hospital meetings and coordinate with hospital social worker.
Coordinate and monitor activities such as a job, school, volunteerism, and family caretaking.
Provide referral to appropriate community resources.

We are Hopeful you have
Bachelor's degree in a human services (Social Work, Psychology, Sociology, Criminal Justice ) field from an accredited institution degree required.
Prior case management or rehabilitation in a mental health or rehabilitation setting preferred.
