Provides community based clinical, case management and consultation services for individuals with mental illnesses, substance abuse and/or developmental disabilities and their families. Also provides supportive counseling, individual and family therapy, crisis intervention, assessment, ISP development, psycho-education, and referrals to clients and their families. Develops and maintains working relationships with: individuals served, families, additional service providers, and other community agencies as needed to assure quality services. Works as part of a treatment team to assure services and resources are coordinated among providers as needed to help individuals served live and function optimally in the community. QUALIFICATIONS Master’s degree in human services or related field is required. In addition, at least one year’s clinical experience working with individuals with substance use disorders, serious mental illness and co-occurring disorders is required. Must be Licensed Eligible or currently hold License as a Professional Counselor or Clinical Social Worker. Certified Substance Abuse Counselor preferred. QMHP eligibility or status is required.
