Behavioral Health Consultant
Philadelphia, PA Territory
The Behavioral Health Consultant functions as part of a multidisciplinary team to improve the health of patients with the most intense and complex medical needs. This position will provide a combination of in-home and telephonic assessment, diagnosis, and therapeutic support to members with documented or suspected behavioral health conditions. This position will collaborate with Tandigm Medical Directors, Nurse Practitioners, Nurse Navigators, Social Workers, patients and their families, and other healthcare professionals to address the patient’s behavioral health needs.
Essential Functions:
- Perform initial comprehensive behavioral health assessments for patients and conduct additional patient assessments as necessary in order to accurately capture symptoms, determine Behavioral Health (BH) diagnoses or conditions, and/or measure and evaluate progress toward goals;
- Collaborate with Clinical Teammates to develop a comprehensive care plan that addresses the patient behavioral health needs;
- Coordinate directly with patient and their care providers to discuss changes in condition, potential medication changes, and modifications to care plan;
- Implement interventions to support the patient to regain or maintain behavioral well-being and monitor the effectiveness of prescribed interventions;
- Document all patient encounters and assessments in designated EMR according to established departmental protocols (i.e., within 24 hours of encounter and per all other guidelines);
- Participate in MDRs and other clinically focused meetings with Clinical Teammates, serving as experts for behavioral health resources to the team;
- Maintain a working knowledge and familiarity with local behavioral health providers and community based resources for patients;
- Participate in the development of clinical practice guidelines regarding patient behavioral health conditions;
- Work with BH Teammates to develop and present educational programs in support of Tandigm Teammates and PCPs and office staff;
- Collaborate with other clinical and administrative staff in a positive, constructive manner;
- Use, protect, and disclose patients’ protected health information (PHI) only in accordance with Health Insurance Portability and Accountability Act (HIPAA) standards;
- Must be able to travel within the five county Philadelphia, PA territory as needed. Must also be able to attend on-site company and department-wide meetings.
- Performs additional duties as assigned or requested.
Job Specifications:
Education/Licensure:
- Bachelor’s Degree from an accredited four-year college along with a Master’s Degree in Psychology, Social & Behavioral Sciences, Nursing, Behavioral/Mental Health, Social Work, or Counseling;
- Valid and unrestricted professional clinical license issued by the Commonwealth of Pennsylvania in the field of social work, human services, psychology, counseling, or nursing, such as Licensed Clinical Social Worker, License Professional Counselor, Clinical Nurse Specialist, or Advanced Registered Nurse Practitioner;
- Valid and unrestricted PA driver’s license
Experience:
- At least five (5) years of professional clinical experience providing behavioral health services to adults and older adults, including but not limited to, patient assessments, diagnoses, and delivery of therapeutic interventions, both in the home as well as telephonically;
- Experience in providing integrated behavioral health services consisting of targeted assessment and evaluation, triage, brief intervention, and/or referral, and acute crisis management;
- Experience working in integrated care environment or other non-traditional mental or behavioral health settings;
- Experience in working in a multidisciplinary, team-based approach to patient care;
- Demonstrated proficiency in oral and written communications, including clinical documentation;
- Trained in social, systems and familial aspects of psychological and medical conditions; Trained in substance abuse identification and treatment with basic knowledge of pharmacotherapy.
Knowledge, Skills, Abilities:
- Understanding of the relationship between medical and psychological aspects of health and disease;
- Demonstrated ability and commitment to deliver consultative services in a responsive, succinct, efficient, ethical and professional manner;
- Ability to provide effective triaging and referral methods to refer patients to resources and specialty mental health care when appropriate;
- Maintains thorough clinical documentation in the patient’s electronic medical record, following departmental and organizational guidelines for timeliness and thoroughness of documentation;
- Completes assessments with patients expressing various issues, e.g. substance abuse, mental health, and psychosocial concerns that may be impacting functioning and quality of life;
- Provides timely and succinct consultation to the HC team or other provider regarding consultation findings and professional recommendations;
- Performs telephone consultation to patients as necessary, including initial assessment/intake, crisis intervention, education and referral, and brief intervention;
- Completes timely follow-up calls to patients where necessary;
- Performs and/or assists with delivery of a variety of screening tools;
- Maintains patient confidentiality as expected by ethical standards, and department and organization guidelines;
- Must be able to work independently and prioritize with minimal direction;
- Must have extensive knowledge of behavioral health resources in the Philadelphia area;
- Must have excellent organizational and planning skills;
- Commitment to act and speak in accordance with the Tandigm Health mission and values;
- Must have valid PA driver’s license, automobile/insurance, and be willing and able to work itinerantly, covering the Philadelphia, PA territory and covering patient assignments by car.
